Patient / Visitor Blue Badge Parking Permit Request

This form is for the completion of patients and visitors who are already in the possession of a valid Blue Badge. Holders of a valid Blue Badge are entitled to free parking on our hospital sites. By submitting your details including an image of your valid Blue Badge within this form, you will be provided entry by Automatic Number Plate Recognition into the multi-storey car park at St. James's University Hospital - please ensure that you also display your valid Blue Badge.

Blue Badge users can also park within any available designated parking bay within our open car parks across Leeds General Infirmary, Chapel Allerton Hospital, Seacroft Hospital, Wharfedale Hospital and St. James's University Hospital whilst displaying their valid Blue Badge.

Please note that your information will be retained on the Automatic Number Plate Recognition system until the expiration date of your Blue Badge.

When you are issued your replacement Blue Badge,
please resubmit your information.
