Before you spend time filling out an inquiry, we want to make sure it is safe medically for you to get a tattoo. If any of the following describes you, we unfortunately cannot tattoo you for your own safety:This question is required.

- You have a medical condition that weakens your immune system (Cancer, HIV/AIDS, emphysema, etc)
- You are taking medication that weakens your immune system (blood thinners, antibiotics, etc)
- You are pregnant or breastfeeding

If you have diabetes, we will need a doctor's note indicating it is safe for you to get tattooed.
