To better serve our community and create an inclusive environment, Oakville Hospital Foundation has set up this short survey to listen and learn more about you.

The mission of Oakville Hospital Foundation is to connect generosity to world-class community healthcare at Oakville Trafalgar Memorial Hospital (OTMH). To ensure our hospital remains a state-of-the-art healthcare facility, Oakville Hospital Foundation raises funds for essential medical equipment and technology required by OTMH healthcare providers to diagnose, treat and care for patients living in our community.

We believe that each of our voice is vital in shaping the future of our Foundation and, by extension, the community we serve. This survey aims to understand how Oakville Hospital Foundation can build more inclusive giving environment for you and our community.

How Your Input Helps
Your feedback will directly inform our strategies for donor engagement, communication, and community outreach. By understanding your experiences and perceptions, we can identify areas for improvement and investment. This ensures that our Foundation remains a welcoming space for all, regardless of background or giving capacity.

Confidentiality and Data Use
This survey is intentionally designed to be anonymous. That means your responses will only be used for the purposes of this research. You will have the option to choose if you would like to self-identify and store your response in your records within our database. Findings from the survey will be analyzed and presented report to our leadership. We value and respect your privacy and will not share this information with any third party not associated with the Foundation.
