Do you consent to TransActual using the information you provide for the purpose of:
1. Identifying and tracking patterns of GP refusals to prescribe HRT
2. Reporting on any patterns we identify.
3. Raising any identified patterns or issues with the NHS and with specific Integrated Care Boards and Health Boards.
4. Contacting you to ask follow-up questions about the issues you experienced.

All of your information will be anonymised. If we intend to contact your local Integrated Care Board or Health Board, we will let you know in advance. If there are fewer than 5 people in your area who have had the same experience as you, we'll get your permission before contacting them.
