Please note that only individuals who have a valid Manitoba Health Card or health coverage through the Interim Federal Health Program (IFHP) or Canadian Armed Forces (CAF), and an address within the WRHA are eligible for care with WRHA midwives.
Please visit the College of Midwives website if you do not live in Winnipeg and would like a midwife.
Previous clients may be eligible for care even if they do not live in Winnipeg.

Please do not complete this form if you have already left a message on our intake line. Completing this form more than once will not result in a quicker response. Someone will respond to you within 2 - 4 weeks. If you are able to, you can come fill out a form in person at Ode'imin (603 St Mary's Road) during regular hours of operation.

Please note that completing this form does not guarantee a spot with midwifery care as capacity is limited.
