MAID Conversion Form

This is the final step in the claim process for converting your MAID to Autonomi Network Tokens. You should have completed the previous steps outlined in the conversion guide.

This form will require the following:

✅ ETH wallet address where you want to receive your ANT tokens, copied from Step 1 — which should be the same address used to sign the message in Step 2
✅ Signed message signature you copied at the end of Step 2
✅ Omni address you sent your MAID from in Step 3
✅ Burn Transaction Hash you saved the end of Step 3

Note: This information will be use by the Autonomi Foundation only to validate and process your airdrop of ANT, and will be held for no longer than 30 days after the airdrop transaction is completed, after which it will be securely deleted.
